All Lone Tree Foods Producers agree to the following:

I have product liability insurance of at least $1 million per occurrence.

I have the ability to maintain the appropriate cold chain from harvest through delivery.

I will provide Lone Tree Foods with access to my farm for initial and follow-up visits, and access to my input records if requested.

I do not use additional labor in my business that is compensated at a rate lower than the current minimum wage (with the exception of on- farm family labor, interns, or barter arrangements).

I encourage energy, soil, and water conservation and health.
I have read and agreed to all Lone Tree Foods producer policies as of the date of this affidavit.

Producers of animal products agree to the following:

I have or am actively pursuing humane livestock certification and/or I am in compliance with humane certification rules, including the following:

•No tail docking, gestation crates, or subtherapeutic use of antibiotics and hormones.
•Ready access to fresh water and a diet to maintain full health and vigor.
•An appropriate environment including shelter and a comfortable resting area.
•Freedom from pain, injury and disease by prevention or rapid diagnosis and treatment.
•Freedom to express most normal behavior by providing sufficient space, proper facilities, and company of the animal’s own kind.

All products have been processed at facilities with the appropriate inspection protocol (USDA or state).

I use only non-GMO feed.

Produce growers agree to the following:

I have completed GAPs food safety training or the equivalent, and will provide a certificate of completion and a written copy of my farm food safety plan.

I use only non-GMO seeds and transplants.
Please check one of the following to describe your growing practices:

____ I use only OMRI (organic) approved inputs and practices, but am not currently certified organic.

____ I use conventional (non-organically approved) inputs and practices sparingly and as part of an IPM management plan.

____ I use conventional (non-organically approved) inputs and practices as my standard production system, based on accepted recommendations.

I understand that any changes or exceptions to the above may only be made by the management of Lone Tree Foods, LLC at their sole discretion.
